Often as Carers you will need to know health information about the person you care for. You might want to know about their medication, their test results, information about their condition, etc.

As patient’s confidentiality is of the utmost importance to us, we train our staff to only give out information to the patient themselves or to another person with the patient’s consent. Sometimes, if we don’t have signed consent this can be frustrating for the carer who needs to know specific information to help look after the person they care for and they may need to know that information quickly.

So that we can make sure you get the information you need, it helps us a great deal if you can make sure that you and the person you care for have completed one of information sharing agreement forms. You can get these at your health centre or email the practice and ask for one to be sent to you. Once completed you can return the form to the Health Centre. Staff will then make a note on the patient’s records that they are allowed to give you information relating to the person you care for.
